Empowering Connection is currently seeking feedback on our NDIS and Disability services to ensure we are continually improving. We would like to invite all current and former participants to complete a short feedback survey.

This feedback survey is optional, and you can choose to remain anonymous. This information will be used to measure the effectiveness of Empowering Connection’s services and Continually improve our systems, processes, and procedures.
