Remember You may be required to make multiple
SIRN is a Serious Incident Report - This includes Death of participant, sexual assault of a participant, Assault of a Participant, or other serious incidents
Child protection or abuse report is for suspected abuse towards a child.
Whistleblower report is for staff to complete if they believe there is misconduct or illegal activity by an Empowering Connection staff member or executive team member.
Tier 1 incident is an incident that required immediate medical attention or posed a serious risk to the health and safety of others
Tier 2 incident is to notify Empowering Connection or an incident or near miss that did not require immediate medical attention or pose a serious risk
Medication incident reports is for medication errors
