Our Social and Community Support services are designed to help seniors maintain active, fulfilling lives by fostering meaningful connections and providing opportunities for engagement.

Our Social and Community supports encompass a range of services aimed at promoting social interaction, community involvement, and overall mental and emotional well-being. These services include:

• Social Activities

• Community Engagement

• Companionship

• Transportation Services

• Volunteer Opportunities, and

• Support Groups

Eligibility:

This service can be provided:

• on its own through the Commonwealth Home Support Programme, or

• with other services as part of a Home Care Package
